Steps for Interscholastic Sports Participation Clearance

Step #1: Complete the online Registration Forms found on Genesis under "Forms"

Step #2: Print and complete the following:

• Health History Form (completed by the parent, brought to the doctor and kept on file at the doctor's office)

• Physical Examination Form (completed by the doctor and kept on file at the doctor's office)

• Medical Eligibility Form (completed by the doctor, and given back to the parent to upload to Genesis)     

Step #3: Upload the Medical Eligibility Form to the Genesis parent portal.

Step #4: Complete the Health History Questionnaire Update (Spring Running Club) in Genesis

Directions for Completing Health History Form in Genesis

• Log into the Genesis Parent Portal 

• If you have multiple children, use the drop-down menu to select the correct child

• Select Student Data>Forms

• Select Health History Questionnaire Update (Spring Running Club), Submit Form 

Directions for Uploading the Medical Eligibility Form in Genesis

• Log into the Genesis Parent Portal

• If you have multiple children, use the drop-down menu to select the correct child

• Select Student Data>Forms

• Select Bogert Medical Eligibility Form Upload

• Please upload forms only after all are completed & signed.

 Physical examination must be conducted within 365 days of the first day of tryouts/practice.

If a child has had an injury requiring medical intervention since the last physical examination, then a medical clearance to participate in sports activities is required.
